Collecting a DNA Sample

Fill out one form per person (only do one test at a time)

 

Fill out the DNA form completely

 

On the outside of the Envelope ~ yellow/brown in color, fill in clients name (you or who ever is being tested), date of birth, collectors initials, if this is you then put your initials, and the collection date.  Once you are done swabbing the inside cheek(s) with the Q Tips they will be placed inside this envelope and sealed.

 


Put on the gloves.    

 

 

Use one swab for each side of the mouth.  Open wrapper at the end opposite from the cotton swab tip and remove one swab.  Do not allow the cotton tip to touch any surface (table, skin, etc.). Doing so could contaminate the sample.

Insert the swab into the mouth and rub it firmly back and forth and up and down while rotating against the inside of the cheek for 10 strokes.

 

It is important to collect enough cheek cells on the entire swab, so be sure to rotate the swab while rubbing.

Insufficient samples may require a recollection and will delay test results.

Remove the swab from the mouth with out touching anything with the tip of the swab
Place the swabs in the sample envelope provided. 

 

Envelope ~ yellow/brown in color labeled for that person and seal.

 

CAUTION: DO NOT PLACE THE SWABS BACK INTO THE ORIGINAL WRAPPER. Moisture created within the paper/plastic sleeve encourages the growth of mold, which destroys the DNA sample.

 

 

 

You will put the DNA Parentage Test sheet and the yellow/brown envelope (with Q tips enclosed) into the white envelope (labeled ~ One specimen collection kit) and seal.   This is per person, per white envelope.

 

 

 

When you are done testing one person, then start over and follow the same steps to do a second DNA test using a new kit.  After you are finished, all the white envelopes should be put into the UPS shipping envelope and shipped out the same day.

 

 

 

Mail everything to the laboratory using the self addressed envelope provided via UPS (prepaid over night shipping) 

·         All the envelopes have a peel and stick closure, do not wet or use your tongue to moisten and seal the envelopes.

Keep samples at room temperature until you can ship it out (same day if possible)
